·     All Experience Certificate from House Job till    to date.
·     Nursing Diploma
·     PNC Card
·      Experience Certificate for Nursing
·   Photocopies of all related documents (one copy each)
·   MBBS Degree duly verified by issuing institute / HEC
·   FCPS Degree duly verified by HEC
·   Attested affidavit of Rs.20 for NRS.
·   Photocopy of NIC (one copy)

·         Application form.
·         Letter of contract.
·         MBBS Degree
·         Valid PMDC registration
·         Valid copy of CNIC.
·         Valid Copy of Passport
·         Fresh Affidavit
·         Valid PMDC registration duly verified by PMDC
